Team Roster and Waiver Form

Each team will need to fill out a team roster and waiver forms before playing. Each player must sign a waiver form. Players 16 or 17 years old will need parent signature. Send bowe.fertig@yahoo.com an email if you need these documents sent as JPEGs.
